North Star Family Medicine is happy to announce we are partnering with Vytalize, an Affordable Care Organization (ACO). An ACO is a group of doctors who partner with one another and health care organizations to provide optimal care for Medicare patients.
As a member of ACO, North Star Family Medicine is rewarded by Medicare for being proactive with our patient’s health and wellness. We ensure referrals placed are to preferred organizations and clinics who offer the same amazing service we provide here. The objective of the team is to coordinate care with the goal of preventing duplicate testing, availability to patients when needed, and reducing costs by decreasing hospitalizations.
