CNP/PA delivers primary medical care to patients from newborn to geriatric patients. They are responsible for maintaining accurate medical records, responding to patient phone calls, and providing follow up care. The CNP/PA is an intricate part of the North Star Family Medicine Team.
Responsibilities:
- Preform patient screenings, obtain patient history of present illness and review of systems, and complete physical examinations.
- Update patient medical records at each appointment.
- Order, interpret and evaluate appropriate diagnostic tests to assess patient’s clinic problems and healthcare needs.
- Formulate an assessment and plan for patient’s
- Review labs and imaging reports in a timely manner
- Refer patients to specialists as needed
- Discuss and coordinate care with other health care providers as needed
- Prescribes appropriate medications, therapy and treatment plans based on the patient’s healthcare needs
- Treat acute and chronic disease
- Perform all medically related assigned duties
- Cooperate with staff and administration for improvement of the medical practice
